Client Bill of Rights and Responsibilities
Client’s Rights
- Every client shall have the right to courteous and respectful interactions. Wise Life Counseling considers your well-being and provide services which best meet your individual and family’s needs in an ethical and professional manner.
- Every client has the right to privacy and confidentiality. No information concerning the client’s involvement in services shall be made available to any organization, agency, or individual outside of Wise Life without your written consent, except as outlined in the Notice of Privacy Practices.
- Every client can expect complete and current information concerning their diagnosis, treatment, and progress in terms they can understand. To ask questions about your treatment; to seek out a second option.
- Every client shall have the right to expect reasonable continuity of care.
- Every client shall have the right to participate in the planning of their treatment, and right to refuse treatment, to include a family member or other chosen representative in the planning of treatment. To seek assistance regarding a grievance of service or behavior or your therapist and her behavior.
- Every client shall be free from mental, emotional, and physical maltreatment and nontherapeutic restraints except in emergency situations and when necessary to protect the client from injury to themselves or to others.
Patient Responsibilities
- You are responsible for being considerate of other clients and staff.
- You are responsible for keeping appointments. A 24-hour cancellation notice is required or a late charge or no-show fee may be applied to your account.
- You are responsible for fulfilling the financial obligations of your health care as properly as you are able.
- You are responsible for loss or damage to clinic property or injury to staff.
Grievance Protocol
If you believe your rights have not been upheld you are encouraged, but not required, to discuss this with your therapist. If a grievance is not resolved with Wise Life Counseling, you have the right to file a complaint with the State of Minnesota Department of Human Services. (651) 296-3971
